dc.description.abstractDenne rapporten presenterer beregninger av antall sprøytemisbrukere i Norge med noe tilleggsinformasjon om alders- og kjønnsfordeling, samt geografisk utbredelse. Blant narkot ikamisbrukere er sprøytemisbrukere antatt å være en spesielt hardt bela stet gruppe, ved at de har en høyere sykelighet og dødelighet enn andre r usmiddelmisbrukere og i samfunnet for øvrig. Sprøytemisbrukere er ofte mer sosialt forkomne og synes også å ha større problemer med selve avhengigheten til stoffene enn andre misbrukere. Å kjenne antall sprøytemisbr ukere er av sentral betydning når behandlingsapparat og hjelpetilbud skal dimensjoneres og ved evaluering av tiltak som er ment å påvirke forbruket av narkotika. For å beregne antall sprøytemisbrukere anvendes tre ulike metoder; en ”dødelighetsmultiplikator”, en undersøkelse på kommunenivå i Norge og en ”flerindikatormetode”. Disse er valgt ut på bakgrunn av hvilke data som er tilgjengelige her i la ndet. Det finnes relativt få gode indikatorer for beregning av sprøytemisbruk og utviklingen i antall over tid. For 2004 antar vi at det var mellom 8 200-12 500 sprøytemisbrukere i Norge. Øverste grense for anslaget ligger noe over det som fremkommer ved bruk av dødelighetsmultiplikatorm etoden, men lavere enn beregninger basert på kommuneundersøkelsen og flerindikatormetoden. De to siste metodene har trolig for høye ansl ag. Indikatorene vi har brukt i beregningene er relativt stabile fra 2004 til 2005, og vi antar derfor at antall sprøytemisbrukere også i 2005 li gger i intervallet 8 200-12 500. Beregninger basert på dødelighetsmult iplikatoren gir en økning i antall aktive sprøytemisbrukere i perioden 1997-2001, fulgt av en nedgang til 2003 og en utflating i 2004/2005. Dermed er det færre aktive sprøytemisbrukere nå enn ved årtusenskiftet. Økningen i antall pasienter i legemiddelassistert rehabilitering (LAR) kan ligge bak nedgangen fra 2001 til 2003. Økningen i LAR fortsatte imidlertid i 2003 til 2005, uten at dette har gitt grunnlag for en fortsatt nedgang i antall sprøytemisbrukere. Dette kan skyldes forhold som at personer i LAR faller ut av rehabiliteringen og tilbake til sprøytemisbruk eller at rekr utteringen har økt. dc.description.abstractThis report details estimates carried out to assess the number of injecting drug users in Norway, including prevalence by age, sex and place of residence. Injecting drug users are widely considered to constitute one of the most vulnerable segments of the drug using population, exhibiting higher mortality and morbidity rates compared with other drug users and the wider public. They are more socially distressed, and problems associated with substance dependency appear to be more severe than among other drug users. Information on the size of the injecting population is essential for capacity planning in the health and social services and for interventions aimed at mitigating drug use. Three methods are used to estimate the size of the injecting population: a mortality multiplier; a community-level survey; and a multiple-indicator method. They were selected in light of available Norwegian data. There are not many good indicators for estimating prevalence and trends in the injecting population. Our estimates suggest that the 2004 population of injecting drug users in Norway ranged between 8,200 and 12,500. The upper limit is slightly higher than that obtained with the mortality multiplier method. The estimates provided by the other two methods are probably over-generous. As our indicators are relatively stable for the 2004-2005 period, our inferred estimate of the 2005 population also lies within the 8,200-12,500 range. Estimates derived from the mortality multiplier show a rising number of active injectors 1997-2000, followed by a dip in 2003 and a plateau 2004/5. Our estimates give therefore fewer active injectors today than around the turn of the century. The rise in the number of patients in substitution treatment (LAR) could explain the fall 2001-2003. Patient numbers continued to grow however in 2003-2005, without a concomitant reduction in the injecting population. This discrepancy could be explained either by substitution treatment drop outs, reverting to active drug use, or a higher influx of new injectors. Available data do not indicate, however, rising recruitment. There is a paucity of information on other therapies in Norway targeting drug injectors, but capacity here has probably remained stable while substitution treatment was being established.
